Essential Duties and Responsibilities: - Facilitate individual and group health education sessions for patients and community members.
- Collaborate with outreach, clinical, and population health teams to design and lead wellness initiatives.
- Promote health equity through culturally responsive and size-inclusive education.
- Develop and distribute plain-language educational materials on various wellness topics.
- Coordinate with local organizations and partners to expand access to health and wellness programs.
- Track participation, outcomes, and generate reports to inform programming and improve effectiveness.
- Support patients in overcoming barriers to wellness such as food access, transportation, and health literacy.
- Promote integrative care and refer individuals to primary care, mental health, and community resources.
- Participate in program evaluation and continuous improvement efforts.
- Evening and weekend availability required with advance notice.
- Other responsibilities as assigned.
Qualifications: This role requires a strategic thinker who can navigate the complexities of managing HR across multiple locations while maintaining a focus on organizational goals and employee well-being.
Education & Experience: - Associate degree in community health, mental health, nutrition, or related field, or equivalent work experience.
- Minimum 2 years of experience in health education, wellness programming, or community outreach.
- Project management experience preferred.
Language & Communication: - Fluency in English and Spanish required.
- Strong interpersonal and written communication skills.
- Ability to communicate clearly using plain language.
Technical & Core Competencies: - Proficient in Microsoft Office Suite (Excel, Word, PowerPoint, Publisher).
- Comfortable presenting to small groups and engaging community stakeholders.
- Self-starter with excellent time management and organizational skills.
- Ability to build respectful relationships across diverse backgrounds and lived experiences.
- Knowledgeable about current immigration issues and community health needs.
Salary and Benefits: - Compensation: The annual salary range for this position is $40,000 to $60,000, based on experience and qualifications.
- Benefits: Tapestry 360 Health offers a comprehensive benefits package, including health insurance, dental insurance, retirement savings plans, paid time off, and continuing education. This position may be eligible for the Federal Public Service Loan Forgiveness (PSLF) program.
Tapestry 360 Health is committed to equitable and transparent pay practices. In accordance with the Illinois Pay Transparency Act, we are disclosing the full salary range for this position. This range represents the potential compensation for the role based on experience, tenure, and performance over time.
Most new employees can expect an initial offer within the lower portion of the range, reflecting factors such as prior experience, internal equity, and organizational budget. Salary progression is evaluated regularly to support professional growth and retention.
