Responsibilities: Provide one-on-one patient care in the patient's home, following a detailed care plan provided by the Nurse Care Manager and/or Physician Perform skilled nursing tasks such as administering medications, wound care, and monitoring vital signs Collaborate with the patient's healthcare team to develop and implement an effective care plan Educate the patient and their family on proper care and treatment method Maintain accurate and up-to-date patient records Communicate effectively with patients, families, and healthcare professionals Qualifications: Registered Nurse (RN) license in IL Minimum of 2 years of nursing experience, preferably in home health or hospice care BLS/CPR certification Strong clinical skills and knowledge of medical equipment and procedures Excellent communication and interpersonal skills Compassionate, patient-centered approach to caregiving Ability to work independently and manage time effectively Broad Street Home Care offers a supportive and collaborative work environment and competitive salary.
