About the Job
Our Client is seeking passionate and dedicated Master’s Social Workers (LLMSW or LMSW) to join their dynamic team. With three openings available at locations in Detroit, Eastpointe, and Southfield, this is a unique opportunity to make a meaningful impact on the lives of seniors in need. In this role, you will be an integral part of the Interdisciplinary Team (IDT), delivering holistic, person-centered care that supports participants' mental, physical, and social well-being.

Who You Are
You are a compassionate, team-oriented professional with a Master's degree in Social Work and an LLMSW or LMSW license in Michigan. You bring expertise in Cognitive Behavioral Therapy (CBT), Motivational Interviewing, and case management. Experience working with frail elderly seniors is preferred but not required. You excel in having crucial conversations and thrive in a collaborative, care-focused environment.

Your Role

• Conduct comprehensive bio-psychosocial assessments for seniors.
• Develop and implement individualized care plans utilizing CBT and Motivational Interviewing.
• Address housing, financial, safety, and psychiatric issues through expert case management.
• Facilitate care transitions, discharge planning, and ongoing risk assessments.
• Collaborate with the Interdisciplinary Team (IDT) to communicate care progress and support participants’ goals.

How You Qualify

• Education : Master’s degree in Social Work from an accredited institution.
• Licensure : LLMSW or LMSW license in Michigan (LLMSWs pursuing full licensure welcome).
• Experience : One year of experience with adults is preferred.
• Skills : Proficiency in CBT, Motivational Interviewing, and case management techniques.

Schedule & Work Location

• Full-time, Monday–Friday, 8:00 AM–4:30 PM or 8:00 AM–5:00 PM.
