Role Description The purpose of a Clinical Informatics Specialist (CIS or Medical Scribe) at Oak Street Health is to support our primary care providers with clinical documentation so that they can focus on providing exceptional care to our patients. Scribes assist providers throughout the patient care journey, including planning, observing, documenting, and supporting post-visit steps. These team members serve as clinical documentation assistants, known internally as CISs, supporting accurate, specific, and timely documentation. They become experts in our value-based care model, chronic condition management, ICD-10 and CPT coding, and help identify care gaps. Extensive on-the-job training is provided in clinical workflows, preventative care, population health data, and team-based care. This role is ideal for individuals committed for at least 1-2 years, especially those on pre-med tracks or pursuing careers in health informatics, public health, healthcare administration, or medical coding. Responsibilities Documenting Patient Encounters (~80%) Joining providers in exam rooms to observe and document visits Creating structured notes, including history, assessment, plan, and physical exam Assigning CPT and ICD-10 codes Preparing After Visit Summaries Consulting with providers for documentation accuracy Clinical Documentation Improvement (~10%) Reviewing medical records and supporting documentation improvement Participating in daily huddles and documentation reviews Collaborating with providers on documentation opportunities Administrative Support (~10%) Placing orders and referrals Addressing administrative tasks Supporting the care team with documentation-related responsibilities Other duties as assigned Qualifications Knowledge of medical terminology and medications (required) Prior clinical experience, shadowing, or volunteering (strongly preferred) Scribe or transcription experience (preferred) Excellent listening and communication skills (required) Strong computer literacy (required) Fluency in Spanish, Polish, Russian, or other community languages (required where indicated) Ability to adapt to workflows and learn quickly (required) Typing speed of 70+ words per minute (preferred) Ability to work as part of a team and follow directions (required) Excellent attendance and in-person work capability (required) Commitment of at least 1 year, ideally 2+ years (required) Availability to work 40-45 hours/week during clinic hours (required) Compliance with policies including HIPAA (required) US work authorization (required) Additional Details Weekly hours: 40 Employment type: Full-time Pay range: $17.00 - $28.46/hour Benefits include medical, 401(k), stock purchase, wellness programs, paid time off, flexible schedules, and more.
